Barry Keoghan has offered fans a behind-the-scenes look at his skincare routine, appearing in a new social media video shared by U.K. celebrity facial expert and Dermaspa founder Nilam Holmes.
The footage shows the Oscar nominee undergoing his second Xerf skin-tightening treatment. According to Dermaspa’s website, Xerf is an injection-free procedure that uses radio-frequency technology to tighten skin and renew collagen. A face and neck session costs approximately $3,500.
Holmes said the video came together while she was running late filming content about the treatment. Rather than have her rush, Keoghan agreed to let her record his own session.
“I’ve been working with Barry for a while now, looking after his skin through different movies, campaigns and projects and the trust he places in me means a lot,” Holmes wrote alongside the video.
She added that Keoghan’s skin has “really improved over the last few years” and said he “looks great.”
Keoghan describes treatment as “relaxing and therapeutic”
Keoghan narrates the video himself, explaining that Holmes’ enthusiasm for Xerf helped generate his interest in the treatment.
“I’ve been hearing a lot about it, and Nilam is very excited about it, and she’s great at what she does,” he says. “I really mean that, and again, every time I come in, it’s very relaxing and therapeutic.”
The actor also connects the appointment to a broader approach to personal well-being.
“I feel better about myself, I look better,” Keoghan says. “I think it’s important to look after yourself inside and outside.”
Holmes has previously posted about treating Keoghan, including giving him facials before photo shoots. In March 2025, she discussed the skincare sessions she provided around his Wonderland magazine cover.
At the time, Holmes said Keoghan’s workload across movies, campaigns, interviews, press and philanthropy projects could involve an unstructured routine, travel, makeup and disrupted sleep. She said those demands could affect his skin, leading them to hold regular appointments focused on problematic areas, inflammation, maintenance and achieving a healthy, rested appearance.
Holmes also described Keoghan as curious about the process, writing that he likes to understand everything and “brings the fun when he visits.”
Keoghan is one of several public figures featured on Holmes’ social media pages. Her other clients shown undergoing Xerf treatment include Chris Appleton, Rosie Huntington-Whiteley, Joe Alwyn and Nicole Scherzinger. Holmes has also worked with Pink and Alwyn.
Video follows Keoghan’s candid comments about online abuse
The skincare video arrives after Keoghan spoke earlier this year about the effect that hostile online remarks about his appearance had on him. During an interview with SiriusXM’s “The Morning Mash Up,” he said the “abuse” had reached the point where he did not want to go outside.
“There’s a lot of hate online,” Keoghan said. “There’s a lot of abuse of how I look, and it’s kind of past the point of — you know, everyone goes through that… but it’s made me shy away.”
He said the comments had caused him to retreat inward and avoid attending places, describing the situation as “a problem.”
Keoghan also explained that he had removed himself from social media to avoid cruel comments. However, he said he would sometimes return online after a premiere or another event to find out how he had been received. According to Keoghan, the response was usually “not nice.”
Nothing in the new video directly links his Xerf appointments to those online remarks. Instead, Keoghan presents the treatment as part of looking after himself and speaks positively about the experience and his trust in Holmes.
ShowBiz Take
Keoghan’s participation makes the clip more than a standard treatment demonstration. It gives viewers a rare look at an ongoing professional skincare relationship that Holmes says has continued across his movies, campaigns and other projects.
His narration also keeps the focus on how the appointment makes him feel, not simply how it changes his appearance. That distinction carries added weight following his candid account of the anxiety and withdrawal caused by online criticism.
By allowing the session to be filmed, Keoghan is sharing one part of his self-care routine on his own terms. Holmes, meanwhile, is clear that this was his second Xerf treatment and part of a longer history of regular skincare sessions—not a one-off visit created solely for social media.
